Under Eye Filler (Tear Trough) in Chicagoland

Under eye filler is a hyaluronic acid gel placed into the hollow beneath the lower lid to replace volume the face has lost. The groove people call the tear trough is not a stain and it is not a bag. It is a shadow, and the shadow exists because the tissue on either side of a fixed ligament has changed while the ligament itself has stayed put.

Three things happen at once with age. The orbital septum, the membrane that holds the fat behind the eye in place, thins and lets that fat sit further forward. The deep fat pads beneath the muscle and in the medial cheek lose volume. The malar fat pad, the soft tissue of the upper cheek, drifts downward. The tear trough ligament does not move at all, so a groove appears along the orbital rim where the tissue above and below it has changed around a fixed anchor. Light falls into that groove, and what people read in the mirror is tiredness.

Filler works on exactly one part of that picture: the missing volume. Hyaluronic acid is a sugar molecule your body already makes, formulated here as a soft gel. Placed underneath the hollow, it raises the floor of the groove so light no longer falls into it. It does not reposition fat, it does not tighten skin, and it does not change pigment sitting in the skin above it.

The target is the infraorbital hollow: the groove running from the inner corner of the eye outward along the lower orbital rim, and the flatter trough that continues toward the cheekbone. That is the whole treatment area. Under eye filler is not placed in the lid itself, at the lash line, or in the skin above the lash line.

Volumes here are small. Published real world data on hyaluronic acid filler around the eye puts the average at just over half a milliliter per side, and most under eye work sits between roughly 0.2 and 0.6 mL a side. That is a fraction of what the same syringe would deliver in a cheek. Injectors also deliberately under correct on the first pass, because hyaluronic acid draws water and the area gains a little more volume over the following days.

Depth matters more here than almost anywhere else on the face. The gel goes deep, onto the bone, beneath the muscle, rather than up near the surface. Deep placement puts the gel as far as possible from the thin skin, which is what stops it showing through as a bluish line, and it keeps the needle or cannula away from the more superficial vessels. Many injectors use a blunt tipped cannula rather than a sharp needle in this area for the same reason: a blunt tip pushes a vessel aside instead of piercing it.

There is one place filler should not go, and it is the reason the assessment carries more weight here than the product does. If the bulge under your eye is orbital fat pushing forward through a weakened septum, rather than a hollow, then the area is not missing volume. It has too much in one spot. Adding gel in front of or below a bulge that already protrudes makes the whole area heavier and more prominent, not smoother. Filler cannot move fat backward and it cannot take fat away. Telling a hollow from a bulge is a physical examination, done in changing light and with your head at different angles, and it is the first thing your injector does.

The appointment is booked for 30 minutes. Most of a first visit is assessment rather than injection, because the decision about whether to inject at all, and where, gets made in front of a mirror with you.

Your injector starts by looking at the area under different lighting and asking you to tilt your head, which separates a shadow cast by a hollow from a bulge that stays prominent whatever the light does. They review your medical history, anything you take that affects bleeding, and any filler you have had in the area before, including filler placed years ago, because hyaluronic acid can persist far longer than most people expect.

If you go ahead, the skin is cleaned thoroughly and a topical numbing cream is usually applied. The gel is then placed through a small entry point, either with a blunt tipped cannula threaded under the skin or with a fine needle depositing tiny amounts directly onto the bone. Either way the amounts are small and go in slowly. Injecting slowly and at low pressure is itself a safety measure, and it is one reason this treatment takes longer than the volume of product would suggest.

You will see a change straight away, along with some swelling. Most people go back to their day. Makeup stays off the entry points for the rest of that day, and you leave with a cold compress and a short list of things to avoid over the next 48 hours.

A follow up at one to two weeks is advised. That is not a formality. It is the point at which swelling has gone, the gel has taken up its final water content, and both of you can see the actual result. If a small amount more is needed, or if something needs adjusting, that is when it happens.

You see a difference in the mirror before you leave, and the honest caveat is that what you see on day one is not the result. Part of it is swelling from the injection itself, and hyaluronic acid attracts water over the following days, so the area keeps changing for about two weeks.

The usual sequence: an immediate lift of the shadow, some puffiness and possibly bruising over the first few days, a stretch around days three to seven where the area can look slightly overfull, then a settled result by about two weeks. Two weeks is the point worth judging the treatment on, and it is why the follow up appointment sits there rather than earlier.

Under the eye, results hold for a long time compared with the more mobile parts of the face. Lips move constantly and metabolize filler quickly. The area under the eye barely moves at all. A real world study following patients treated with hyaluronic acid filler around the eye found the effect still present at twelve months, with roughly one in six choosing a touch up and fewer than one in ten needing a full retreatment inside that year.

Around a year is a reasonable expectation, and a good number of people go longer. Individual results vary, and how long yours lasts depends on how much was placed, how quickly your body clears it, and what you are comparing it against.

The result is not permanent. Hyaluronic acid is broken down and cleared by the body over time, and that is the same property that makes it correctable.

The comparison that matters is not between filler brands. It is between filler and the treatments that address the other reasons an under eye area looks dark.

Filler compared with surgery. Lower blepharoplasty removes or repositions the orbital fat that has pushed forward through a weakened septum. Filler adds volume into a hollow. Those are opposite operations on opposite problems, and filler placed to camouflage a genuine fat bulge tends to read heavier rather than smoother. Filler compared with treatments for pigment. If the darkness under your eye is brown rather than blue or grey, and it barely changes when you tilt your head toward a light, it is more likely to be pigment sitting in the skin. Filler is not a cure for pigment driven darkness, because the gel sits below the layer the pigment lives in. Topical treatment, peels and light based treatment work on that layer instead. Establishing which one you have is worth doing before anyone books an injection.

Filler compared with waiting. A genuine option, particularly where the hollowing is mild or the picture is mixed. Nothing about this treatment gets harder for having waited.

Reversibility is the real argument for a hyaluronic acid filler in this area. Hyaluronidase, an enzyme that breaks the bonds holding hyaluronic acid chains together, dissolves the gel within about 24 to 48 hours. It is the clinical gold standard for managing a filler problem, whether that is a lump, a bluish cast, an overfilled look, or an emergency. Fillers that are not hyaluronic acid cannot be reversed this way, which makes them a harder starting point in an area that is among the higher risk sites on the face.
