Hyaluronic Acid Dermal Filler

Perioral Line Filler

Perioral lines are the small vertical lines that form above and below the lips, and the creases that develop at the corners of the mouth. They are sometimes called smoker's lines or lipstick lines, though you do not have to smoke to develop them. They form as the skin around the mouth loses collagen and the underlying support structure thins with age, and they tend to make lipstick feather and the mouth area look older than the rest of the face. A small amount of precisely placed hyaluronic acid filler can smooth these lines and restore definition to the lip border without overfilling or distorting the natural shape of the mouth. At Express Med Spa, our nurse practitioners treat this area with a conservative, detail-oriented approach because the zone around the lips has very little margin for error and a lot of impact on the overall result.

RESULTS TIMELINE

Results are visible immediately. Swelling in the first 24 to 48 hours may make the area appear fuller than the final result. Optimal, settled appearance is typically seen at the one to two week mark.

Result Last

6 to 9 months on average for the perioral area. The mouth is a high-movement zone, which means filler in this area tends to metabolize faster than in lower-movement areas such as the cheeks. Patients who also incorporate a neurotoxin treatment for the lip area may find their results last toward the longer end of that range.

  • Stefanie Drozd, Full Practice Authority Nurse Practitioner specializing in aesthetic and wellness treatments

How does perioral line filler work?

A small amount of hyaluronic acid filler is placed in the skin around the mouth to smooth the fine vertical lines and restore definition at the lip border, without adding volume to the lips themselves.

  • Stefanie Drozd, Full Practice Authority Nurse Practitioner specializing in aesthetic and wellness treatments

    Which areas are treated?

It targets the vertical lines above and below the lips, sometimes called smoker's or lipstick lines, along with the lip border and corners of the mouth. The lip body itself is treated separately with lip filler.

  • Stefanie Drozd, Full Practice Authority Nurse Practitioner specializing in aesthetic and wellness treatments

    What is the appointment like?

The visit runs about 20 to 40 minutes. Because this area is delicate, the filler contains lidocaine and topical numbing is applied first, so most patients find it very manageable, with minimal downtime.

  • Stefanie Drozd, Full Practice Authority Nurse Practitioner specializing in aesthetic and wellness treatments

When will I see results, and how long do they last?

Results are visible right away and settle over one to two weeks. The mouth is a high-movement zone, so filler here lasts about 6 to 9 months, toward the longer end if paired with a little neurotoxin.

  • Stefanie Drozd, Full Practice Authority Nurse Practitioner specializing in aesthetic and wellness treatments

    Perioral filler or lip filler?

Perioral filler smooths the skin and lines around the mouth; lip filler adds volume inside the lips. They address different concerns, and many patients treat both in the same visit.

    About This Treatment

If fine lines around your mouth or feathering along the lip border are bothering you, here is how perioral line filler smooths them, which areas it treats, what the appointment involves, how long it lasts, and how it differs from lip filler, so you know what to expect.

    Treatment Pricing

Perioral line filler is priced per syringe, though this area typically requires less product than larger zones such as the cheeks or nasolabial folds, so many patients achieve a good result with a partial syringe. Your final cost is based on the volume your provider recommends, and remaining product can often be applied to a connected area such as the lip border or marionette lines in the same visit. Members receive a discounted rate, and monthly payment plans are available through CareCredit and Cherry. Your provider will map out a personalized volume plan at your consultation.

Who Perioral Line Filler is For

Good Candidates? Is This Right for You?

Perioral line filler is appropriate for adults 22 years and older who have visible vertical lines above or below the lips, creasing at the corners of the mouth, or a loss of definition along the lip border that makes the area look older or causes lipstick to feather. These concerns are most common in patients in their forties and beyond, though patients who smoke, have significant sun exposure history, or have naturally thinner skin around the mouth may notice perioral lines developing earlier.

This treatment works best for patients with mild to moderate perioral lines who want a subtle smoothing result rather than a dramatic change. Patients with deeper static lines, significant skin laxity in the area, or very active lip movement may see partial improvement with filler alone and may benefit from a combined approach that also addresses the area with a small amount of neurotoxin. Your nurse practitioner will assess the full perioral zone during your consultation and give you an honest picture of what filler can and cannot achieve for your specific anatomy.

This is a technically demanding treatment area due to the thinness of the skin, proximity to the lip border, and high degree of facial movement. All perioral filler at Express Med Spa is performed by a licensed nurse practitioner or registered nurse injector with appropriate training in this zone.

      What To Expect

      Pre & Post Treatment Care

      Pre-Care Instructions
Avoid blood-thinning medications and supplements for 48 to 72 hours before your appointment if medically safe to do so. This includes aspirin, ibuprofen, naproxen, fish oil, vitamin E, and alcohol. Do not stop any prescription medication without consulting your prescribing physician first.
Avoid alcohol for at least 24 hours before your appointment.
Arrive with clean, makeup-free lips and surrounding skin. Remove all lip products and topical treatments from the perioral area before your appointment.
Stay well hydrated and eat a light meal before your visit.
If you have a history of cold sores or oral herpes simplex virus, you must disclose this before booking. Any injection near the mouth can trigger a herpes simplex reactivation. Your provider may recommend a short course of antiviral medication before your appointment as a precaution.
Do not schedule perioral filler within 4 weeks of laser resurfacing, intense pulsed light, chemical peel, or dermabrasion at or near the treatment area.
Inform your provider of all current medications, supplements, recent dental procedures, and any changes to your health.
If you have active dental work, an oral infection, or a scheduled dental procedure within two weeks of your appointment, notify your provider before booking.

      Post-Care Instructions

Do not rub, massage, or apply any pressure to your lips for at least 24 hours following treatment.
Avoid lip products for the remainder of your treatment day, including lipstick, lip gloss, lip liner, and balm. You may resume lip products the following day.
Avoid kissing, using straws, and any activity requiring significant lip puckering for 24 to 48 hours as the filler settles.
Avoid strenuous exercise and activities that significantly elevate your heart rate for 24 hours after your appointment.
Avoid alcohol for 24 hours post-treatment to minimize bruising and swelling.
Avoid sleeping face-down on the night of your treatment. Sleep on your back if possible.
Avoid significant heat exposure for 24 to 48 hours, including saunas, steam rooms, hot showers, and prolonged sun exposure.
Mild swelling is expected and typically most noticeable in the first 24 to 72 hours. A cold compress applied gently can help reduce swelling, do not press or massage the lips.
Allow a full two weeks before evaluating your result. Swelling in the first few days can make your lips appear fuller than the final settled outcome.
Contact Express Med Spa immediately if you experience a cold sore outbreak, severe pain, skin discoloration, or any symptoms that concern you after treatment.

      • How Perioral Line Filler Works

Perioral line filler is a non-surgical injectable treatment that uses a small amount of hyaluronic acid gel to smooth the vertical lines that form around the mouth and to restore definition along the lip border. It targets one of the more telling signs of facial aging in a zone where the skin is thin, highly mobile, and very visible.

The filler is placed in the skin surrounding the lips rather than in the lip body, so it softens lines and rebuilds structure without adding volume to the lips themselves. Because the area has very little margin for error, our nurse practitioners take a conservative, detail-oriented approach.

      • Areas Treated

Perioral line filler addresses the small vertical creases that form directly above and below the lips, sometimes called smoker’s lines or lipstick lines, along with the lip border and the corners of the mouth where the skin can begin to crease or turn slightly downward.

This is distinct from the lip body itself. Adding volume, projection, or shape to the lips is done with lip filler, which uses different placement. The two treatments target different areas and are often planned together when a patient wants to treat both the lines around the mouth and the lips.

      • What to Expect at Your Appointment

A perioral filler appointment usually takes 20 to 40 minutes. Because this is a sensitive, technically demanding area, comfort is a priority: Revanesse Versa+ contains lidocaine, and topical numbing cream is applied beforehand, which is especially helpful around the upper lip. Most patients describe the experience as very manageable.

Downtime is minimal, though the area can swell or bruise in the first 24 to 48 hours. If you have a history of cold sores, let your provider know before booking, since any injection near the mouth can trigger a flare and a short course of antiviral medication may be recommended as a precaution.

      • Results Timeline and How Long It Lasts

Results are visible immediately, though swelling in the first 24 to 48 hours can make the area look fuller than the final result. The settled appearance is typically seen at the one to two week mark.

The perioral area is one of the higher-movement zones of the face, so filler here tends to metabolize faster than in areas like the cheeks, with most patients seeing results last 6 to 9 months. Patients who also use a small amount of neurotoxin to relax the repetitive muscle movement that drives these lines often find their filler results last toward the longer end of that range.

      • Perioral Filler vs. Lip Filler, and Reversibility

The key distinction is placement. Perioral line filler is placed in the skin around the mouth to smooth lines and restore the lip border. Lip filler is placed inside the lip body to add volume and shape. Many patients choose to do both in the same appointment, treating the lines around the mouth and adding fullness to the lips together.

Perioral filler can also be paired with a small amount of neurotoxin above the upper lip to soften the muscle movement that deepens these lines over time. And because Versa+ is a hyaluronic acid filler, it can be dissolved with hyaluronidase if you are ever unsatisfied with the result, the same as filler placed anywhere else on the face.
