Perioral Line Filler

Perioral lines are the small vertical creases that form in the skin directly above and below the lips, and the folds that develop at the corners of the mouth. They are sometimes called smoker’s lines or lipstick lines because of the way lipstick tends to bleed into them. Despite the nickname, you do not need to smoke to develop them. They form as the skin in this area gradually loses collagen, the lip border loses definition, and the repeated movement of the mouth over decades compresses the surrounding skin. Sun exposure and smoking can accelerate the process, but for most patients the main cause is simply time.

Lip filler adds volume inside the lip body itself, increasing fullness, projection, and shape. Perioral line filler is placed in the skin surrounding the lips, specifically in the fine vertical lines above and below the lip border and at the corners of the mouth. The two treatments address different concerns and use different placement techniques. Many patients choose to do both in the same appointment, which allows the provider to plan the volume across the entire lip and perioral zone together for a more balanced result. Perioral line filler is not intended to add volume to the lips themselves, so it should not significantly change the apparent size of your lips. The goal is to smooth the lines in the surrounding skin and restore definition at the lip border, not to project or inflate the lip. The filler formula contains lidocaine, a built-in local anesthetic, which helps reduce discomfort during injection. Topical numbing cream is applied before treatment and is especially helpful in this area. Most patients describe the experience as manageable, with brief moments of sharpness at the needle entry points.

Any injection near the mouth can potentially trigger a herpes simplex reactivation in patients with a history of oral cold sores. Your nurse practitioner may recommend a short course of antiviral medication taken before your appointment as a precaution. The perioral area is one of the higher-movement zones of the face, and filler here tends to metabolize faster than in areas like the cheeks or nasolabial folds. Most patients see results lasting 6 to 9 months. Patients who also use a small amount of neurotoxin in the perioral area to reduce the repetitive muscle movement that causes these lines may find their filler results last toward the longer end of that range.

Yes, and the two treatments are often used together. A small amount of neurotoxin placed carefully above the upper lip can relax the muscle movement that causes radial lip lines to form and deepen over time. This does not freeze the lips but subtly reduces the compression that drives the lines. Used in combination with filler, the result can be more complete and longer-lasting than either treatment alone.
