Perioral lines are the small vertical lines that form above and below the lips, and the creases that develop at the corners of the mouth. They are sometimes called smoker's lines or lipstick lines, though you do not have to smoke to develop them. They form as the skin around the mouth loses collagen and the underlying support structure thins with age, and they tend to make lipstick feather and the mouth area look older than the rest of the face. A small amount of precisely placed hyaluronic acid filler can smooth these lines and restore definition to the lip border without overfilling or distorting the natural shape of the mouth. Results are visible immediately. Swelling in the first 24 to 48 hours may make the area appear fuller than the final result. Optimal, settled appearance is typically seen at the one to two week mark.

Result Last

6 to 9 months on average for the perioral area. The mouth is a high-movement zone, which means filler in this area tends to metabolize faster than in lower-movement areas such as the cheeks. What is Perioral Line Filler?

Perioral line filler is a non-surgical injectable treatment that uses a small amount of hyaluronic acid gel to smooth the vertical lines that form around the mouth, restore definition at the lip border, and soften the creases at the corners of the lips. It is a precise, detail-oriented treatment that targets one of the more telling signs of facial aging in a zone where the skin is thin, highly mobile, and very visible.

If you have noticed fine vertical lines above your upper lip, faint creases below the lower lip, lipstick that bleeds into the surrounding skin, or corners of the mouth that have begun to crease or turn slightly downward, this is the treatment designed to address those concerns.

Treatment Pricing

Price shown is per syringe. The perioral area typically requires less product than larger treatment zones such as the cheeks or nasolabial folds. Many patients achieve a good result using a partial syringe for the radial lip lines and oral commissures alone. Remaining product from the same syringe can often be applied to a connected area such as the lip border or marionette lines in the same visit, depending on your goals. If you are also booking a lip filler treatment at the same appointment, your provider will map out a combined volume plan during consultation. Perioral line filler is appropriate for adults 22 years and older who have visible vertical lines above or below the lips, creasing at the corners of the mouth, or a loss of definition along the lip border that makes the area look older or causes lipstick to feather. These concerns are most common in patients in their forties and beyond, though patients who smoke, have significant sun exposure history, or have naturally thinner skin around the mouth may notice perioral lines developing earlier.

This treatment works best for patients with mild to moderate perioral lines who want a subtle smoothing result rather than a dramatic change. Patients with deeper static lines, significant skin laxity in the area, or very active lip movement may see partial improvement with filler alone and may benefit from a combined approach that also addresses the area with a small amount of neurotoxin. Your nurse practitioner will assess the full perioral zone during your consultation and give you an honest picture of what filler can and cannot achieve for your specific anatomy.

This is a technically demanding treatment area due to the thinness of the skin, proximity to the lip border, and high degree of facial movement. All perioral filler at Express Med Spa is performed by a licensed nurse practitioner or registered nurse injector with appropriate training in this zone.

Pregnancy or breastfeeding
Active cold sore, open wound, or skin infection at or near the planned injection site (note: patients with a history of oral herpes should discuss prophylactic antiviral coverage with their provider before treatment in this area)
Known allergy to hyaluronic acid, lidocaine, or streptococcal proteins
History of severe allergic reaction (anaphylaxis) or heightened immune response to common allergens
History of keloid scarring or hypertrophic scarring
Active autoimmune condition or connective tissue disorder such as systemic lupus erythematosus
Current anticoagulant or antiplatelet therapy, or personal history of a bleeding or clotting disorder
Under 22 years of age
Laser treatment, intense pulsed light, chemical peel, or dermabrasion at the treatment site within the past 4 weeks
Unhealed wound or active inflammatory skin condition such as rosacea or seborrheic dermatitis at the planned treatment area

Risks / Side Effects

Swelling, bruising, bleeding, redness, and mild pain at the injection site, common and expected; typically resolves within a few days to two weeks depending on the treatment area.
Acne-like skin eruptions at or near the injection site.
Skin sensitivity including rash, itching, and tenderness at the injection site.
Skin infection at the injection site.
Skin lumpiness or irregularity at the injection site, often temporary as the product integrates with the surrounding tissue over the first one to two weeks.
Hyperpigmentation (skin darkening) at or near the injection site.
Damage to nerves or blood vessels in the treatment area.
Scarring at the injection site.
Skin necrosis (tissue death), rare but serious; contact your provider immediately if you notice skin blanching, mottling, or unusual pain after treatment.
Reactivation of herpes simplex infection (cold sores or skin sores), patients with a history of oral herpes should disclose this before booking any lip or perioral treatment.
Warning: Unintentional injection into a blood vessel is a risk with any dermal filler injection. Vascular complications can include vision abnormalities, blindness, stroke, temporary scabs, or permanent scarring of the skin. What To Expect

Pre & Post Treatment Care

Pre-Care Instructions

Avoid blood-thinning medications and supplements for 48 to 72 hours before your appointment if medically safe to do so. This includes aspirin, ibuprofen, naproxen, fish oil, vitamin E, and alcohol. Do not stop any prescription medication without consulting your prescribing physician first.
Avoid alcohol for at least 24 hours before your appointment.
Arrive with clean, makeup-free lips and surrounding skin. Remove all lip products and topical treatments from the perioral area before your appointment.
Stay well hydrated and eat a light meal before your visit.
If you have a history of cold sores or oral herpes simplex virus, you must disclose this before booking. Any injection near the mouth can trigger a herpes simplex reactivation. Your provider may recommend a short course of antiviral medication before your appointment as a precaution.
Do not schedule perioral filler within 4 weeks of laser resurfacing, intense pulsed light, chemical peel, or dermabrasion at or near the treatment area.
Inform your provider of all current medications, supplements, recent dental procedures, and any changes to your health.
If you have active dental work, an oral infection, or a scheduled dental procedure within two weeks of your appointment, notify your provider before booking.

Post-Care Instructions

Do not rub, massage, or apply any pressure to your lips for at least 24 hours following treatment.
Avoid lip products for the remainder of your treatment day, including lipstick, lip gloss, lip liner, and balm. You may resume lip products the following day.
Avoid kissing, using straws, and any activity requiring significant lip puckering for 24 to 48 hours as the filler settles.
Avoid strenuous exercise and activities that significantly elevate your heart rate for 24 hours after your appointment.
Avoid alcohol for 24 hours post-treatment to minimize bruising and swelling.
Avoid sleeping face-down on the night of your treatment. Sleep on your back if possible.
Avoid significant heat exposure for 24 to 48 hours, including saunas, steam rooms, hot showers, and prolonged sun exposure.
Mild swelling is expected and typically most noticeable in the first 24 to 72 hours. A cold compress applied gently can help reduce swelling, do not press or massage the lips.
Allow a full two weeks before evaluating your result. Swelling in the first few days can make your lips appear fuller than the final settled outcome.
