Marionette line filler is a non-surgical injectable treatment that uses a hyaluronic acid gel to soften the creases that form at the corners of the mouth and run downward toward the chin. These lines develop as facial volume naturally decreases with age, pulling the lower face downward and often creating a resting expression that reads as sad or stern. A licensed nurse practitioner places a small amount of filler precisely along the fold to restore that lost volume, lifting the area and neutralizing the downward pull. The appointment typically takes 30 to 45 minutes, results are visible the same day, and most patients see them last 9 to 12 months.

Marionette lines are commonly treated using a partial syringe, which means the remaining product from that syringe can often be applied to a related area such as the oral commissures or nasolabial folds in the same visit. Patients who want to address both the marionette lines and nasolabial folds comprehensively may benefit from a second syringe. This concern tends to appear or worsen in the mid-thirties and beyond as volume in the lower face decreases naturally with age, though some patients notice it earlier due to genetics or facial structure.
This treatment is particularly effective for patients whose marionette lines give them a resting expression that reads as sad, tired, or stern. Mild to moderate lines typically respond well to a single treatment session. More pronounced folds or patients who want a more comprehensive lower face result may benefit from also treating the nasolabial folds or oral commissures in the same visit, which your provider can assess during consultation.
