Dysport vs Botox: What Actually Differs, and Why Units Are the Part People Get Wrong

Read this first: Express Med Spa does not offer Dysport. Our four Chicagoland clinics carry Botox® Cosmetic, Xeomin®, and Letybo®. We wrote this comparison because the question gets asked constantly and most of the answers online are wrong about the one thing that matters most, which is unit dosing. If you have decided on Dysport®, you will need a provider who carries it. Nothing below is an attempt to move you off it.

Dysport and Botox Cosmetic are both botulinum toxin type A injectables. They relax the same muscles by the same mechanism, they are used for the same expression lines, and in trained hands they produce results most patients would struggle to tell apart in a mirror.

The differences are real but narrow. They live in formulation, in how the product behaves in tissue after it is injected, in how quickly it starts working, and above all in how each product’s units are counted. That last one is not a technicality. It is the reason a Dysport price quote and a Botox price quote can look wildly different while describing nearly the same treatment.

Here is the short version, then the detail behind it.

  • Same drug class, different formulation. Both are botulinum toxin type A. Botox Cosmetic is onabotulinumtoxinA. Dysport is abobotulinumtoxinA. The active molecule is the same toxin; the proteins packaged around it differ.
  • Units are not interchangeable. Roughly 2.5 Dysport units are used where 1 Botox unit would be. A per-unit price comparison between the two is meaningless on its own.
  • Dysport is often described as spreading a bit wider. This is genuinely debated, and dilution and injection technique influence it at least as much as the product does.
  • Onset may be slightly faster with Dysport. Duration is broadly similar, commonly three to four months for both.
  • Neither is stronger or safer than the other. They are dosed differently, not ranked.

What is the actual difference between Dysport and Botox?

Both products contain the same active ingredient class: botulinum toxin type A. Injected into a muscle, the toxin blocks the release of acetylcholine at the neuromuscular junction. Without that signal, the muscle contracts less forcefully. The overlying skin creases less, and the expression line softens. The effect is temporary, and it wears off as nerve signaling is restored.

That mechanism is identical between the two. What differs is what else is in the vial.

Botulinum toxin is produced naturally as a complex: the active neurotoxin bound to a set of non-toxic accessory proteins. Manufacturers handle that complex differently.

  • Botox Cosmetic is supplied as a toxin complex with its accessory proteins intact, in a relatively large and consistent complex size.
  • Dysport is a different toxin complex from Botox Cosmetic. Clinic literature often describes it as a mix of smaller complexes with a lower accessory protein load relative to its potency. That description is commonly repeated and is not a labeled dosing fact; treat it as background, not as a reason to pick one product over the other.
  • Xeomin, which we do carry, is processed to remove the accessory proteins entirely, leaving the neurotoxin alone. It is sometimes described as a bare or purified formulation.
  • Letybo, which we also carry, is a botulinum toxin type A product that retains accessory proteins.

The practical relevance of accessory protein load is smaller than marketing on all sides suggests. The theory is that a lower protein load presents less foreign material to the immune system, which could reduce the chance of developing neutralizing antibodies that make future treatments less effective. Antibody formation with modern cosmetic dosing is uncommon, and how much the formulation difference actually changes that risk in cosmetic use is not settled. It is a reasonable factor to discuss with your injector if you have been treated for many years or have noticed your results fading faster than they used to. It is not a reason for a first-time patient to choose one product over another.

Why are Dysport units not the same as Botox units?

This is the single most misunderstood point in the entire comparison, and it causes real confusion about pricing.

A “unit” of botulinum toxin is not a measure of weight or volume. It is a measure of biological activity, and each manufacturer establishes it using its own potency assay. A Dysport unit and a Botox unit are both units of biological activity, but they were defined by different tests against different reference standards. They are not the same size.

The prescribing information for these products states this directly: potency units are specific to each preparation and the assay method used, and they cannot be converted to or compared with units of any other botulinum toxin product.

The clearest way to see the scale of the difference is to look at the approved dosing for the same treatment area. For frown lines between the brows, which clinicians call the glabellar complex, the FDA-approved regimens are:

  • Botox Cosmetic: 20 units total, given as 4 units at each of five injection points.
  • Dysport: 50 units total, given as 10 units at each of five injection points.

Same area. Same five injection points. Same intended result. The unit counts differ by a factor of 2.5.

That 2.5 to 1 relationship is the conversion most injectors work from, and published ratios generally fall somewhere between 2 to 1 and 3 to 1 depending on the area and the clinician. It is a working guide, not a fixed conversion constant, and it is one reason dosing is set by a clinician who knows the product rather than calculated from a chart.

The takeaway for a patient is simple and worth stating plainly: if someone tells you they received 50 units, that number means nothing until you know which product it was. Fifty units of Dysport is an ordinary frown-line dose. Fifty units of Botox would be a substantially larger treatment covering multiple areas.

Does Dysport really spread more than Botox, and when does that matter?

Dysport has a longstanding reputation for a wider field of effect, meaning the treated zone extends somewhat further from each injection point. You will see this stated as settled fact on a lot of clinic websites. It is not settled.

Here is an honest read of the evidence. Some comparative studies do show a broader area of effect for Dysport at comparable doses. Others find the difference small or inconsistent. And a large portion of the variation that gets attributed to the product is actually driven by two things the injector controls:

  • Dilution. Both products are reconstituted with saline before injection. More diluent means more volume per unit of toxin, and more volume spreads further. A clinic that reconstitutes Dysport in a larger volume will see more spread, and that is a technique decision, not a property of the drug.
  • Injection depth, placement, and volume per point. Where the needle goes and how much fluid is deposited at each site changes the result more than the brand on the vial does.

Where spread genuinely matters is in the trade-off it creates.

A wider field of effect can be useful across a broad, uniform muscle. The forehead is the usual example: the frontalis is a large flat sheet, and a product that covers more ground per injection point can smooth it evenly with fewer needle sticks.

A more contained field of effect is generally preferred wherever precision protects something nearby. Around the eyes, near the brow, and in the lower face, small muscles sit close to other small muscles that you very much want to leave alone. Unintended spread near the upper eyelid can affect the muscle that lifts the lid and cause a temporary droop. Unintended spread in the lower face can affect a smile. These are recognized, temporary, and uncommon effects, but they are the reason experienced injectors treat delicate areas conservatively regardless of which product they are using.

This is also why “which product spreads more” is a less useful question than it sounds. A skilled injector adjusts dilution, dose, and placement to get the field of effect the area needs. The product is one input among several.

How fast does each one work, and how long does it last?

Onset is the one area where a modest, fairly consistent difference shows up.

  • Dysport is commonly reported to begin working within two to three days.
  • Botox Cosmetic is commonly reported to begin working within three to five days.

Both continue to develop and reach their full effect at roughly two weeks. That two-week mark is when your injector can fairly assess the result and decide whether any adjustment is needed, and it is the number that actually matters for planning around an event. A day or two of difference at onset is a genuine distinction, but it rarely changes a treatment decision on its own.

Duration is broadly similar. Both products commonly last about three to four months for cosmetic use. Individual duration varies considerably based on the dose used, the area treated, muscle strength, activity level, and metabolism. Patients with strong, frequently used facial muscles often find any neurotoxin wears off sooner. Claims that one of these products reliably outlasts the other are not well supported once dosing is properly matched.

Results vary from person to person, and no injectable outcome can be guaranteed.

Which product is typically chosen for which area?

There is no rule that assigns a product to an area, and any clinic that tells you there is has oversimplified. What exists is a set of tendencies that follow from the spread characteristics above.

Larger, broader areas. The forehead is the area most often cited as a reasonable fit for a product with a wider field of effect, because the muscle is large and the goal is even softening across it.

Small, precise areas. Crow’s feet at the outer corners of the eyes, brow shaping, a lip flip, and fine work in the lower face all reward a tightly controlled field of effect. Here the priority is treating one small muscle without affecting its neighbors.

Strong, bulky muscles. Masseter treatment for jaw slimming or clenching involves a large, powerful muscle and higher dosing. Product selection here follows the injector’s dosing experience more than any spread characteristic.

In practice, most injectors develop fluency with a small number of products and get consistently good results across all of these areas with them, by adjusting dose, dilution, and technique. Familiarity with the product in the hands treating you is worth more than the theoretical profile of a product that clinician uses rarely.

Is Dysport cheaper than Botox?

Usually not, once you compare the same thing. This is where the unit issue turns into a pricing trap.

Most clinics that price neurotoxin per unit will list a lower per-unit figure for Dysport than for Botox. That looks like a discount. It is not, because the units are different sizes.

Work through the arithmetic on a frown-line treatment:

  • A Botox frown-line dose is about 20 units.
  • The equivalent Dysport dose is about 50 units.

If a clinic charges roughly 40 percent as much per Dysport unit as it does per Botox unit, and it takes 2.5 times as many units, the two treatments cost close to the same. That is exactly why per-unit prices for the two products tend to sit where they do. The per-unit number is scaled to the unit size.

So the question to ask a clinic is never “what do you charge per unit.” It is:

  • “What will this treatment area cost me in total?”
  • “How many units of which product does that include?”

Those two answers are comparable across clinics and across products. A per-unit rate on its own is not. Be especially cautious with any advertised per-unit price that does not name the product, and with promotional pricing that quotes a unit rate without saying how many units your treatment requires.

What does Express Med Spa offer instead of Dysport?

We carry three botulinum toxin type A products:

  • Botox Cosmetic, the product most patients arrive asking for by name.
  • Xeomin, the purified formulation without accessory proteins. Patients who have received the same product for many years and feel their results have become less reliable sometimes switch to it.
  • Letybo, a more recently approved option that some patients choose on price or on their injector’s recommendation.

We do not carry Dysport, and we are not going to tell you that makes Dysport a worse choice. It is a widely used, FDA-approved product with a long track record. It is simply not on our shelf, and we would rather say that plainly than have you book a consultation expecting it.

If the comparison you are actually weighing is a different pairing, each one has its own guide rather than a single roundup, because the answers genuinely differ: Xeomin vs Botox and Letybo vs Botox.

Which product you receive, how many units, and where they are placed are decided during your consultation based on your anatomy, your treatment history, and what you are trying to change. These are prescription products, and suitability is determined at that consultation.

Can you switch between Dysport and Botox?

Yes, and it is common. Patients move between botulinum toxin products for all sorts of reasons: they change clinics, a product becomes unavailable, pricing shifts, or they want to try something different because their results have felt inconsistent.

A few practical points:

  • No washout period is generally needed. You do not have to let one product fully wear off before receiving a different one, though your injector will usually want to treat on your normal schedule rather than stacking treatments close together.
  • Your dose will be recalculated, not carried over. This is the unit issue again. Tell your new injector which product you received and how many units. If you only remember the number, say that you are not sure of the product. A good injector would rather start from your treatment history and your muscle strength than from a unit count that might belong to a different scale.
  • Give the new product a fair trial. Assess at two weeks, not at day three. Onset differences between products can make a switch feel slower or faster than it really is.
  • Tell your injector what you did and did not like. “It wore off too fast,” “my forehead felt too heavy,” and “I could not raise my brows” are all useful and all fixable with dose and placement adjustments, usually regardless of which product is used.

What are the risks, and are they different between the two?

The side effect profiles of these products are broadly similar, which follows from them being the same class of drug.

Common and temporary effects include injection-site redness, small bumps that settle within an hour or two, bruising, and headache. Less common effects relate to the toxin affecting a muscle other than the intended one, which can produce a temporary eyelid or brow droop, an uneven smile, or asymmetry. These resolve as the product wears off.

Botulinum toxin products carry a boxed warning regarding the possibility of effects spreading from the injection site, which can cause swallowing and breathing difficulties. This has been reported primarily in patients treated with much higher therapeutic doses for medical conditions, not at cosmetic doses, but it applies to the class and it belongs in any honest discussion of these products. Seek medical attention for any swallowing, speech, or breathing difficulty after treatment.

These treatments are not appropriate for everyone. Pregnancy, breastfeeding, certain neuromuscular conditions, active infection at the injection site, and some medications are all relevant, and there are others. Is Dysport the same as Botox?

Is 50 units of Dysport the same as 50 units of Botox?

Can I switch from Dysport to Botox without waiting?

Either can work well. The honest bottom line

For most people, Dysport and Botox Cosmetic are close enough that the choice will not determine whether you are happy with your result. Dosing accuracy, injection placement, and a clinician who listens to what you actually want changed will determine that.

What is worth carrying away is the unit issue. It is the one real, concrete, frequently-exploited difference between these two products, and understanding it protects you from a pricing comparison that looks like a bargain and is not.
