How Long Does Botox Take to Work? A Day-by-Day Timeline

Lily Zelaya
October 20, 2026

Botox usually starts to work within a few days, but it typically takes about two weeks to reach its full result. Many people notice the first softening around day 3 or 4, the effect keeps building through the second week, and by about day 14 most patients can see what their dose has done. That two-week mark is also the point at which it makes sense to judge whether anything needs adjusting.

Prefer to watch first? In a 91 second video on Botox touch-ups at day 7 versus day 14, Lily Zelaya, RN, Aesthetic Nurse Injector at Express Med Spa, explains why she waits for the full 14 days before adding any units. This guide covers the rest of the timeline: what typically happens on each day, why the result takes about two weeks, why one side can kick in first, who Botox suits, and how long it lasts once it settles.

How Long Does Botox Take to Work?

Expect the first change within a few days and the full result in about 10 to 14 days. Cleveland Clinic notes that most people notice the desired effects of Botox around the third or fourth day, with full results visible within 10 to 14 days. Our own Botox treatment page describes the same pattern: results appear gradually over 3 to 14 days, and most patients see their full result by day 14.

Some people feel it sooner. In a 2011 study of onabotulinumtoxinA for frown lines (onabotulinumtoxinA is the drug name for Botox), nearly half of the participants reported onset by day 1, and every participant had reported onset by day 4. The authors described the onset as rapid, at one to two days. Onset is not the same as the finished result, though. The first softening you notice is only the beginning, and the muscle typically keeps relaxing for several more days after that.

So the answer has two parts. Botox usually starts working within a few days, and it usually finishes working at about two weeks. Timing varies from person to person, and from one treatment area to another.

Botox Results Day by Day

This is the general timeline Lily walks through in the video, with the aftercare from our treatment page added for treatment day. Your own timing may run a little faster or slower.

Treatment day

You usually will not see a change yet. Some bruising or swelling at the injection sites can happen, and it typically resolves within 3 to 5 days. Our aftercare asks you to stay upright for 4 hours, avoid touching or rubbing the treated areas for 4 hours, and skip strenuous exercise, saunas, and hot yoga for the rest of the day. Within those limits, most patients return to work and normal activities the same day.

Day 1 to day 2

In the video, Lily describes day 1 as the point where the tox has not kicked in yet and you still have your natural movement. That is a common experience, but not a universal one. In the study above, nearly half of participants reported some onset by day 1. Either way, what you see in these first two days is not your result.

Days 3 to 7

This is when results typically start to show, and it is also when things can look a little uneven. Movement softens, but one side may settle before the other, and some lines may still be visible. Lily is clear that this is not the final result. One side still moving at day 5 does not automatically mean you need a touch-up.

Days 7 to 13

The effect continues to settle. As Lily explains, an area that looks like it needs a couple more units on day 7 may look different by day 14. This is the window where patients are often tempted to ask for more, and it is the reason she does not touch up at day 7.

Day 14

For most patients, this is the full result. The dose has had time to take effect, and your injector can judge what it did. Some unevenness can still remain at this point, and that is normal. Our treatment page lists asymmetry and an effect that is not quite strong enough as things that can be addressed at the 2-week follow-up visit.

Week 3 to month 4

The result typically holds for 3 to 4 months, and movement returns gradually rather than all at once. Many patients plan their next treatment around the 3 to 4 month mark.

Why Botox Takes About Two Weeks to Fully Work

Botox does not fill or erase a line directly. It works on the muscle underneath. Cleveland Clinic explains that Botox blocks the nerve signals that control muscle activity, which relaxes the muscles that cause wrinkles. When the muscle contracts less, the skin over it creases less, and expression lines soften.

That relaxation does not happen in a single moment. It builds over a period of days, which is why the change is gradual and why the first week can look patchy. Our post on what Botox actually does covers the mechanism in more detail, including why it is not a magic eraser for every kind of line.

Is It Normal for One Side to Kick In First?

It can be, especially in the first week. In the video, Lily explains that a lot of what she sees, even in herself, is one side being stronger than the other. Her own left side takes a little longer to kick in than her right. Faces are rarely perfectly symmetrical, so one side of a treated area may still move more than the other for a few days.

Lily’s point is that days 3 to 7 are not the final result, so an uneven look at that stage does not automatically mean you need more units. If a difference is still there at day 14, that is useful information for your injector, and the 2-week follow-up visit is where it is addressed.

What the Two-Week Mark Means for Your Results

Our treatment page lists attending your 2-week follow-up visit as part of Botox aftercare, and touch-ups for dose adjustments are handled at that appointment. Judging the result at about two weeks, rather than one, helps in three ways:

  • You see a settled result. At day 7 the result is usually still changing.
  • Any adjustment can be more targeted. Your injector can see which area, if any, needs more and add units only where they are needed.
  • Your next visit can be better planned. What you needed at day 14 helps your injector plan your dose next time.

If you are a first-time Botox patient, plan on attending your 2-week follow-up visit. If something worries you before then, call your clinic. Lily explains the touch-up side of this in detail in her video on Botox touch-up timing.

Why Adding Units Too Early Can Backfire

The simplest reason is the one Lily gives at the end of the video: more Botox can be added later, but units that have already been administered cannot be taken away. Units added at day 7 stack on top of a dose that may still be taking effect. Once the full result arrives, the area can end up more relaxed than you wanted.

The forehead is one area where this matters. Lily explains that adding units prematurely can leave patients feeling very heavy, especially in the frontalis muscle, the muscle that lifts your eyebrows and creates horizontal forehead lines. The FDA-approved prescribing information for BOTOX Cosmetic lists brow ptosis (a lowered or heavy brow) as a reported side effect of forehead line treatment, at 2 percent of patients in its clinical studies, and directs injectors to treat forehead lines together with frown lines to minimize the potential for brow ptosis. Cleveland Clinic also lists eyebrow droop and eyebrow asymmetry among the possible side effects.

A heavy feeling or a lowered brow usually fades as Botox wears off, but that can take weeks. Waiting until the result has settled before adding units can help lower the chance of overdoing it. If you notice a drooping brow or eyelid, contact your clinic. Our guide to dynamic vs. static wrinkles explains which lines respond to Botox and which may need a different approach.

Do Xeomin and Letybo Work on a Similar Timeline?

Broadly, yes. Express Med Spa offers three wrinkle relaxers, and our treatment pages list similar timelines for each:

  • Botox: results in 3 to 14 days, lasting 3 to 4 months.
  • Xeomin: results in 3 to 14 days, lasting 3 to 4 months.
  • Letybo: results in 2 to 14 days, lasting 3 to 4 months.

Because each one builds over as much as two weeks, a result judged at one week is often still changing, whichever product you choose. Ask your injector when your follow-up should be for the product you receive. In the video, Lily uses “tox” as shorthand for these wrinkle relaxers. If you are deciding between products, our Xeomin vs. Botox comparison covers the differences, and our post on how long Xeomin lasts covers that product’s timeline in more depth.

How Long Botox Lasts Once It Kicks In

Once Botox reaches its full effect, it typically lasts 3 to 4 months. Cleveland Clinic puts the average at about three to four months as well. Movement usually returns slowly, so most people notice lines coming back gradually rather than all at once.

Several things can shorten that window. Our treatment page notes that high-activity areas like crow’s feet may fade slightly faster than the forehead, which moves less throughout the day. Heavy exercise also speeds up your metabolism, which can make neurotoxin wear off faster. Our post on what makes tox fade fast goes through the common causes, and these Botox skincare steps can help you get the most from each treatment.

Spacing matters too. The BOTOX Cosmetic prescribing information notes that the safety and effectiveness of dosing more frequently than every 3 months have not been clinically evaluated. Your injector plans the timing of any adjustment and your next full treatment with that in mind.

Who Is a Good Candidate for Botox?

Our treatment page describes Botox Cosmetic as suited to adults 21 and older who have dynamic wrinkles, the lines that appear or deepen when you make a facial expression. If you can see lines form when you raise your brows, squint, or frown, those are dynamic wrinkles. Common areas include horizontal forehead lines, the vertical “11s” between the brows, and crow’s feet.

Botox is not right for everyone. Our treatment page lists these reasons to wait or to talk it through with your injector first:

  • Pregnancy or breastfeeding
  • A known allergy to botulinum toxin or any ingredient in Botox Cosmetic
  • A neuromuscular disease such as myasthenia gravis, Lambert-Eaton syndrome, or ALS
  • An active infection or inflammation at the planned injection site
  • Current use of blood-thinning medications or supplements, which you should discuss with your injector
  • Recent surgery in the treatment area within the prior 6 months

Share your full medical history, current medications, and any supplements at your consultation, because some conditions and medications need a closer look before treatment.

What to Expect at Your Botox Appointment

A Botox appointment at Express Med Spa is quick. The injections themselves typically take 15 to 20 minutes, with minimal downtime. Your injector reviews your goals, looks at how your face moves, and plans the number of units for each area before treating.

Botox is priced per unit, starting at $10.95 per unit for first-time guests, so your total depends on the areas treated. As our treatment page notes, a common 20-unit frown-line dose lands near $220 at that rate. Pricing varies with the number of units you need and is confirmed at your consultation. Check our current specials before you book.

To reduce the chance of bruising, our treatment page recommends avoiding aspirin, NSAIDs, fish oil, vitamin E, and alcohol for about 7 days before treatment. Never stop a medicine your doctor prescribed without asking that doctor first, and tell your injector about every medicine and supplement you take. If you are new to Botox and wondering when to start, our guide to preventative Botox covers lighter dosing for earlier lines.

Frequently Asked Questions

How long does Botox take to work?

Many people notice the first softening around day 3 or 4, and the full result typically arrives in about 10 to 14 days. Results appear gradually over 3 to 14 days, and timing varies from person to person.

How long does it take to see full Botox results?

For most patients, about two weeks. Express Med Spa considers day 14 the point at which most patients see their full result, which is why follow-ups and any dose adjustments are handled at a 2-week visit.

Is it normal to still see lines one week after Botox?

Yes. At one week Botox is usually still settling, so some lines or movement can remain. Judge the result at about day 14. Lines that stay visible even when your face is at rest may need a different approach, which your injector can discuss with you.

Why is one side of my face working before the other after Botox?

Faces are rarely perfectly symmetrical, and one side can respond sooner than the other. During days 3 to 7 one side may settle first. Any difference still there at day 14 can be addressed at your 2-week follow-up visit.

Why does my forehead feel heavy after Botox?

A heavy feeling can happen when the frontalis muscle, which lifts the brows, is relaxed more than needed. The BOTOX Cosmetic prescribing information lists brow ptosis as a possible side effect of forehead treatment. It usually fades as Botox wears off. Contact your clinic if you notice a drooping brow or eyelid.

How long does Botox last?

Botox typically lasts 3 to 4 months once it reaches its full effect. Movement usually returns gradually, and high-activity areas such as crow’s feet may fade slightly faster than the forehead.

Do Xeomin and Letybo take as long as Botox to work?

They work on a similar timeline. Express Med Spa lists results in 3 to 14 days for Xeomin and 2 to 14 days for Letybo, each lasting 3 to 4 months. Ask your injector when your follow-up should be for the product you receive.

Who should not get Botox?

Botox is not recommended during pregnancy or breastfeeding, with a known allergy to botulinum toxin, with certain neuromuscular diseases, or with an active infection at the injection site. Blood thinners and recent surgery in the area should be discussed with your injector first.
